High Holy Days Honors Request for 5780 (2019)

Welcome to the High Holy Days at Peninsula Sinai Congregation! We are glad that you will be joining us. Please use this form to request honors for the High Holy Days services. 

All requests for honors are due by Monday, August 5, 2019.

Note: Separate registration forms will be available later this summer for Youth Programming and other High Holy Days opportunities (L’Shanah Tovah Card, Yizkor Book, Community Break Fast, etc.). Please note that you MUST also complete the registration form to confirm your attendance at HHD services.

A letter confirming your honors will be sent a few weeks before the holidays. L’shanah Tovah!

All members are encouraged to participate in High Holy Days Services.  If you, or a member of your family, would like to be considered for an honor, please do the following for each person:

• Place a check in the column for each type of honor for which that person wants to be considered
• Place a check in the column corresponding to ALL the days this person will be present at our services

If you have more than one family member who would like to have an honor, please click “Yes” at the bottom of the page to enter requests for each additional member.  The committee tries wherever possible to honor all requests.
